First visit paperwork

What to expect on a first visit

At your first visit, the doctor will:

  • ask you questions about your health history, and
  • perform a physical exam, particularly of your spine.

The initial examination lasts approximately one hour. If you have any records of prior X-rays or MRIs, please bring them in, however the doctor may recommend additional examinations or tests.

Based on these initial exams, our doctor will put together a treatment plan to suit your needs and treatment goals. Please note that the chiropractic treatment recommended may require a series of visits. Ask the doctor about the frequency of visits and be prepared to work them into your schedule. It might be a good idea to bring your calendar along or have an idea of when in your schedule you can fit in visits to the chiropractic office.

Insurance & Payment Information

We accept insurance on a case by case basis. When you come in, please bring your insurance card and we will verify your benefits. The first visit, however, is payable in full at the time of service.

We accept cash and check payments, as well as credit cards: Amex, Discover, Mastercard and Visa.

Online Paperwork

We offer our paperwork online so you can complete it in the convenience of your own home. Download, print the forms and bring the completed paperwork with you on your first visit.
